Objectives

  • Early Diagnosis: facilitate early detection and reduce medical wandering to limit functional disability caused by the irreversible damage of the disease.
  • Continuity: ensure seamless care transitions across multidisciplinary teams, from child to adult, adult to senior, and between city and hospital settings.
  • Integration of Care and Research: leverage current research findings to inform clinical practices, while using clinical observations to shape research and innovation programs. Actively involve patients in driving innovation.
